Please submit your request for malpractice verification along with a signed authorization and release to the Office of GME at firstname.lastname@example.org.
To request a duplicate certificate:
Send a notarized statement.
A notarized statement confirms your identity and protects the confidentiality of your training records. All requests must bear the signature of the Medical Partnership trainee who is requesting the duplicate diploma.
The statement should include:
- The address to which the diploma should be mailed
- Your full legal name
- Last 4 digits of your SSN
- Start and end date of training program
- Send a $50 check payable to University of Georgia (The fee covers the cost of printing and mailing)
